Levels of Care
We offer all levels of the senior care San Diego area families count on. Our home care services are available in La Jolla and throughout the San Diego area and vary from Caregiver/Personal Care Aide level to Highly Skilled RNs.
We will develop a plan that features the nursing and care services that will perfectly meet your needs, at the time you wish, for the duration that is required – all with professionalism and friendliness that makes home care the preferred choice by seniors.
- Caregivers – We provide caregivers, personal care aides and companions for help in Activities of Daily Living, housekeeping, shopping, companionship, escort services, personal care, safety supervision, respite care for families and more.
- Licensed Vocational Nurses – We provide LVNs for all of the caregiver services plus LVN skilled duties.
- Registered Nurses – We provide RNs for all of the caregiver and LVN services plus skilled duties of an RN.

Caregivers: Duties and Responsibilities:
Personal Hygiene:
- Assist patient with personal care and activities of daily living, safety supervision
- Bathing, shaving, hair care, and routine skin care
- Oral hygiene including denture care
- Dressing and undressing
- Prevention of pressure ulcers
- Apply non-sterile, dry dressings to intact skin surfaces
- Empty or change Foley catheter bag
- Empty or change Colostomy bag
- Care of hearing aides, glasses, and prosthetic devices
- Nail care (filing ok, but no cutting nails)
Medical Care:
- Obtain and record vital signs including blood pressure, temperature, pulse and respirations
- Remind or assist with pre-poured medications
- Chart the medical record
- Housekeeping
- Light housekeeping, maintain a clean and healthy environment
Safety:
- Home safety, emergency preparedness, CPR and basic first aid.
Transportation:
- May drive/escort on errands, shopping, doctor appointments
Mobility:
- Assistance with mobility: ambulation, transfers, positioning, and prescribed range of motion exercises
Nutrition:
- Meal planning, preparation, serving, and feeding
- Measure and record “intake” and “output”

Duties of Registered Nurses (RNs)
Medication Administration:
- Administer medications via most routes. This includes pre-pouring medications.
- Administer all IV medications and fluids. May also calculate and adjust IV flow charts
- Use infusion pumps
Patient Care:
- Perform initial patient care plans
- Perform initial or ongoing periodic patient assessments
- Catheterize a patient
- Administer tube feedings
- Perform stoma care
- Perform glucometer tests
- May apply heat and cold
- Suction and care for tracheostomy tube
- Change sterile dressings
- Perform bowel/bladder irrigation
- Perform other diagnostic and therapeutic nursing procedures
- Initiate and maintain written care plans for individual patients
- Educate and instruct patients, patients’ families and staff
- Perform some personal care and household duties, as necessary.

Duties of Licensed Vocational Nurses or LVNs
Medication Administration:
- Administer medications via most routes, including pre-pouring medications
- With IV certification, may administer IV fluids to include electrolytes, nutrients, vitamins, blood and blood products. May also calculate and adjust IV flow rates (nutrition, hydration, and blood products only.)
Patient Care:
- Catheterize a patient
- Administer tube feedings
- Perform -stoma and -ostomy care
- Perform glucometer/blood sugar tests
- May apply heat and cold
- Suction and care for a tracheostomy tube
- Perform Wound Care and change sterile dressings
- Perform bowel/bladder irrigation
- Perform other diagnostic and therapeutic nursing procedures
Care Planning:
- Assist the RN in initiating and maintaining written care plans for individual patients
Patient Education:
- Assist with educating and instructing patients, patients’ families and staff limited to teaching information which is outlined in and is consistent with the vocational nursing practice act.
Household Duties:
- Performs some personal care and household duties, as necessary
